A registered nurse in Evansville was arrested after being accused of stealing controlled drugs from the assisted living facility she worked at.

Jena Grady, 34, was arrested on felony charges of prescription fraud and narcotics possession.

Grady was working at Good Samaritan Home, now called Envive Healthcare of Evansville, when the crimes occurred, according to investigators from the Office of the Indiana Attorney General’s Medicaid Fraud Control Unit.

After receiving alerts about anomalies, the senior care home’s leaders started reviewing Grady’s drug log sheets.

Authorities said the investigation revealed multiple cases in which Grady claimed patients declined opioids or that narcotics were dumped, but there were no witnesses to the waste, as required by the law. They claim that some trash forms contained scribbled and unidentifiable witness signatures.

According to an affidavit, investigators discovered roughly 27 units of unaccounted drugs.

During an interview with Grady, officials claim they confronted her with the fact that one of the nurses indicated that she had witnessed Grady spill 12 tablets in one 12-hour shift, although all of the other nurses said it happened less than once in their entire careers.

Grady was arrested and put into the Vanderburgh County Jail on L6 felony charges of obtaining a controlled substance through fraud or deceit, possessing a narcotic drug, and failing to make, keep, or furnish documents. She was released with a $500 bond.
